Physical therapy forms need to cover the related aspects of client-patient management, initial evaluation / management, re-examination, visits and discharge. Each visit done by the patient needs to be supported by physical therapy forms. The name of the patient and identification number needs to be documented in every form. All the physical therapy forms which are related to evaluation, examination, discharge and visits need to be authenticated by physical therapist providing the service. These forms need to clearly indicate the details like the patient being referred by a doctor or if he has come directly. Physical therapy forms need to mention referral source from where the patients had come down for the visit.

Details in the therapy forms-Evaluation forms or initial examination includes the relevant details like general demographics, employment details, social history, development and growth, living environment, social and health habits, general status of the health, family history, surgical or medical history, primary complaints at the moment, recent medications and various clinical tests conducted. The therapy forms related to each visits need to ask the patients about the following details like, patient’s self report, changes in their present conditions and response to the interventions. The discharge forms need to contain the patients’ present physical status, improvement and additional services that are referred to. Most of the physical therapists tend to ask the patients for filling out the forms depending on the type of insurance they are allowed and injury type. If the therapy forms are covers ‘No fault insurance’ then, the patient needs to fill in the appointment policy form, information form, benefit assignment form, authorization form and history form. Patients who are not covered by the insurance covers need to fill up the form with the details like personal information, health questions, health survey, financial policy, history, authorization and appointment policy.
